View Full Version : Help, my paperwork came back to me...

I got my Dr recommendation in July, mailed everything to the state.

I just received it back because I did not include a valid copy of the picture ID of my caregiver.

The catch is that I designated a dispensary as my caregiver. Per their instructions they had me use the name and address, etc, of the owner. I never had access to his ID, and I think I got caught in that new law July time warp? Did I miss something?

The terrible catch is that the dispensary has since changed ownership(With the new September laws...sigh). I have already signed a change of caregiver form for the new caregiver, and it has been submitted to the state.

I have left a voice mail with the State, after being lost in their endless automated voice system forever. I of course do no anticipate hearing back.

So do I:
A: write a letter explaining all that, and send a notarized copy of the change of caregiver form back in with my completed paper work and hope for the best?

or

B: Is my application now invalid and I need to start at step 1?

Sorry to hear about your predicament, the state has done a wonderful job of making things very confusing and frustrating for the patient. As a caregiver in CO, I would recommend re-sending your application, but with your new caregiver's info. I wouldn't worry a whole lot about the other caregiver form, the same person will be your caregiver, they'll just be listed on both forms, or they'll get the change of caregiver form first, and not have you listed as a patient yet. Who know's what they do with them then. It took me 9 months this past renewal process from when they recieved my application to when I got my card. Hope they get their stuff together before next year. But, I did get 9 months extra, so I'm not complaining too much. Best of luck.

Thanks for your reply. I had come to that conclusion and then ran into the woman who helped with my paperwork when I saw the Dr and she guessed the same thing.

She had not had any other patients call her with that, and she knew she had quiet a few people who chose the same dispensary around the same time and nobody else had called with that question.

Your reply made me more reassured in that I am guessing right on this one. I just am trying to do this right. :) So off to stand in line on my lunch hour tomorrow and get it all right again.

They send back a good number of applications for various reasons. We had one where they wrote an 8 that might look like a 6 if you really stretched it. Sent it back with "illegible ssn"

I think you just need to fill out a new application and have it notarized. Be sure to use the new form and designate MMC instead of caregiver. If you designate a center, you don't have to include an ID. Ship them back everything they sent you. You might be inclined to redo the check if it was a personal check. If a money order maybe cash it and get a new one.

Applications that have been redone seem to get fast tracked... well they come back faster than brand new ones anyways :)
