Grow report template.
Need to know room, closet, or cabinet.
Growing space size length x width x height.
Watts per square foot, which is room length x width divided into total watts. I.e. 5'x5'=25sqf with 1000w light is 25 into 1000=40w per square foot.
Reflective material used, foil, paint, mylar, or other.
Amount of light used in watts, and type, i.e. fluorescent, Hps, Mh, or other.
Ventilation type if any.
Growing method, whether soil, hydroponic. If hydro what type, drip, flood and drain, deep water culture, nft, etc.
Growing style, i.e. double potting, screen of green (scrog), sea of green (sog).
Ferts used, chemical, or organic. Be sure to include brand name if applicable.
Additives, whether you use h2o2, super bud, additional co2, etc.

In the body of the report, you will want to give a good overall idea of your grow and it's progress. You will want to give your germination method, and how long they took. What type water you used while germing(tap, bottled, spring, or rain). Or if by clone, your cloning method, brief description including how long it took them to root. Such details like if you used heating mats, humidity domes, or some other homade contraption.
We then move on to early vegetive growth, some things to note would be as follows. Size of initial pots when placed to veg in. What you fed if anything, how much, or whether you used any additives. List your normal temps, humidity, distance of your light. A reference to how they grew and measurements would be beneficial. You will need to list your training style if any. Also in hydro set up list your water source, and normal temperature, nute schedule etc. Note any odor or insect problems.
After the plant reaches a few weeks old (3-4)(may not apply in sog) as you begin to feed more and the plant really starts to grow, you will need to continue recording as above. Making notes of height, width of plant through measurement. Making sure to list all plant foods (note whether chemical or organic), and amounts used, and frequency. Be sure to note how often you water, and what tips you off to water, or whether you just go by a schedule. If in hydro how do you up your nutes, and any additives or aides. Also be sure to document all problems, disorder, and sickness in plants. This is VERY IMPORTANT, as we seldom learn much from a perfect grow. It is adversity and how we react to this adversity that makes us improves, and teaches us all.
Carry on with your careful observations during the flowering cycle. Continue to make notes and how you handle common problems like stress, early stretching, and high temps, and or humidity. Be sure to note any insect precautions you may have taken, or any infestations, and how you dealt with them. Make sure to note your lighting schedule, and why you use that particular one. Make notes of how the flowers form. How fast they come into flower, how quick they show trichomes, note periods of heavy flowering. Explain the smells as well as possible, and any special things you may have had to do to control them.
Other things you may want to include would be the soil recipe you use, or you selection of nutes for hydro. Give brief explanations of your training styles. An overview of any equipment like screens, wire, trellis or netting used in your training, or the grow in general. Pictures of all stages, and special equipment if possible. ALWAYS be sure to note basic health of plants through out the grow cycle. Be sure to try to analyze your grow and see if you can spot it's weakness, and what you might do to improve them.
The most important thing is to be a complete as possible. These are meant to be used as tools to help folks, Which means the more information the better. Try to be concise but keep your language simple and easy to understand. We are here to help, not confuse or confound. As I already mentioned list ALL problems, and your solutions, how you came about your diagnoses etc. Far to many people hit a hump it there grow and fail to report it, or just quit reporting. DO NOT do this I ask you please. This does all of us a disservice as were learn from mistakes, they are our friends. Theres no reason to be embarrassed we all screw it up once and awhile. I am also sure anyone caught flaming problems will be dealt with summarily.
That's about it. Just be complete, and have fun. Remember this is also of benefit to all, use your head take pride in your work and help out.

Outdoor reports.
Outdoor reports will be a bit more simple. Basically you just need to state whether you started clone or seed, germination, or cloning technique used. When you started, and when you planted it outside. Give an explanation of how you preped your hole, or if you grew in containers how the soil was prepared. It would be benificial if you were to note you latitude, and general climate.
Include all fertilizers, and their application. Report any pest problems, drainage headaches, watering conundrums etc.
Make note of how the plant performs to each situtation. Details of approximate size, any training methods involved. Information pertaining to how much direct light the plant(s) recieve. List any special tools used, or special growth additives involved.
Be sure to mention when the plant begins to flower. How long it takes, how fast formation takes place. Make notes of plant stretch and odor. Include when it finishes, and how it handled cold temperatures (if applicable).
